Intrauterine Insemination

This treatment is offered to some couples with unexplained subfertility, depending on age of female partner and duration of subfertility.

It involves taking a combination of Clomiphene citrate tablets and hormone (gonadotrophin) injections to stimulate the ovaries starting on day 2 or day 3 of cycle. The treatment includes self administration of hormone injections and transvaginal ultrasound scans to monitor response from the ovaries. Usually by day 11 to day 1 there will be development of 1to maximum 3 follicles off 18-20 mm size. At this point ovulation is triggered using hcg injection. Ovulation is expected to occur 36 hours after the hcg injection. Treatment will be strictly abandoned if there are more than 3 mature follicles measuring 14 mm or more to reduce the risk of triplet an higher order multiple pregnancy. The twin pregnancy rate is around 10%.

On that day, the male partner produces a sperm sample in the clinic, which is washed and prepared so that the very motile sperms are collected in a small volume of media . and this is then left inside the uterine cavity using a very fine catheter, in a very simple procedure, similar to a smear test. A pregnancy test is done in 14 days time. If unsuccessful, the procedure can be repeated for a maximum of three cycles. Treatment will be reviewed earlier if necessary
